Inclusive Health premiums vary based on age, gender, whether you are a tobacco user or
not and your choice of plan option (PPO 1000, PPO 2500, and High Deductible
Health Plan 5000). Premium rates are updated on an annual basis.

To calculate your expected monthly premium, enter your age, gender
and tobacco use status in the monthly premium calculator:

Check any of the following statements that apply to you:
     Your medical condition makes you "uninsurable".
     You have been offered coverage by an insurer with a conditional rider limiting coverage.
     You have only been offered coverage at a premium rate that is higher than Inclusive Health's rate.
     You have had 18 months of continuous converage and have exhausted COBRA coverage.

January 1, 2010 Rates - Non-Tobacco Use
January 1, 2010 Rates - Tobacco Use Benefit Plan Options starting Jan 1st.

New Benefit Plan Option
Beginning January 1, 2010, Inclusive Health will be offering a new benefit plan option. An additional PPO plan is being added with a $3,500 deductible. It includes all of the same benefits as the $1,000 and $2,500 deductible PPO plans but with a higher deductible and lower premium rates.

We have also re-named our plans to reflect these changes:
  • PPO 1000 - PPO plan with a $1000 deductible - HIGHEST MONTHLY PREMIUM
  • PPO 2500 -- PPO plan with a $2500 deductible
  • NEW: PPO 3500 -- PPO plan with a $3500 deductible
  • High-Deductible 5000 - High Deductible health plan with a $5000 deductible - LOWEST MONTHLY PREMIUM
The benefit plan chart below shows a complete comparison of the plan options that will be available starting January 1, 2010:



Premium rates are updated on an annual basis. Deductibles are calculated on a calendar - January - December.
A new deductible will begin each year in January.
